Job description
The Licensed Practical Nurse (LPN) provides clinical and administrative support to the PHP program. The LPN is responsible for routine patient health monitoring, documentation, coordination of medical needs, and supporting the daily operations of the PHP site. The successful candidate will be compassionate, dependable, organized, and able to work effectively in a behavioral health setting while maintaining professional boundaries and patient confidentiality.
Essential Functions
Clinical Responsibilities Obtain and document vital signs, weights, and other routine patient assessments. Perform urine toxicology screening (UTox) according to program schedule. Perform Accu-Cheks and other nursing procedures as ordered and within scope of practice. Assess patient concerns and report changes or abnormal findings to the appropriate nurse, provider, or Site Director.
Notify medical providers of abnormal results according to established protocols. Manage Quest laboratory orders and results as assigned. Maintain accurate and timely documentation in Kipu/EHR. Support patient safety and assist with medical needs within the LPN’s scope of practice. Maintain infection control and appropriate PPE practices.
Administrative/Program Responsibilities Assist with new admissions, welcome materials, checklists, and patient orientation. Maintain daily census and attendance information. Assist with no-show follow-up and patient communication as directed. Assist with scheduling, transportation coordination, phone calls, emails, and other daily office needs.
Maintain inventory and assist with ordering supplies. Complete required safety checks, reports, spreadsheets, and other program documentation. Support the Site Director with billing and other administrative duties as needed. Support patients in the milieu as directed. Attend staffings, meetings, and supervisions as assigned.
Maintain a clean, safe, and professional PHP environment. Perform other duties as assigned by the Site Director or Director of Nursing.
Qualifications
Current, unrestricted Illinois LPN license. Graduate of an accredited practical nursing program. CPR/BLS certification or ability to obtain. 1–2 years of nursing experience preferred; behavioral health experience preferred. Experience with electronic health records; Kipu preferred. Strong clinical, organizational, communication, and documentation skills.
Ability to multitask and balance clinical and administrative responsibilities. Ability to maintain confidentiality and appropriate professional boundaries. Must meet all applicable SCH, DASA, CANTS, background check, and Joint Commission requirements. #GMH
