At Select Home Health & Hospice, we believe in putting patients first: “Select the best for your health!” About the Role Select Home Health is seeking a motivated and compassionate Licensed Practical Nurse (LPN) / Account Executive to provide high-quality in-home nursing care while building relationships with healthcare providers and referral sources. This role combines hands-on patient care with sales, community outreach and referral development to support patient well-being and grow our home health services.

Responsibilities

Clinical Nursing Responsibilities Provide skilled nursing care in accordance with physician orders and established care plans. Monitor patient conditions, assess pain and educate patients and families on care needs. Administer medications safely and follow established medication administration procedures. Promote patient safety, confidentiality and quality of care.

Sales & Referral Development Build and maintain relationships with healthcare providers, community partners and referral sources. Educate referral partners about Select Home Health services. Identify opportunities to expand referral networks and increase patient census. Execute outreach plans and track activities in the CRM.

Represent Select Home Health at community events and networking opportunities. Monitor market trends and identify opportunities for growth. Administrative & Team Collaboration Maintain accurate records and complete required reports. Coordinate with clinical teams to support patient needs and referral transitions. Collaborate with leadership on outreach initiatives and agency growth.

Ensure clinical and business development activities comply with applicable regulations and agency policies. Communicate patient status and needs with the interdisciplinary care team. Complete accurate and timely clinical documentation. Participate in required trainings, in-services and meetings. Follow agency policies, procedures and emergency protocols.

Qualifications

Graduate of an accredited nursing program Active state LPN license in the state of Indiana Recent acute care or home health experience (home health preferred) Valid driver’s license, reliable vehicle, and auto insurance Current TB test and CPR certification
