Community-Based Services Therapist

The Community-Based Therapist plays a key role in supporting youth through care transitions into family or family-like settings. Working primarily in the community including family homes, foster homes, group homes, schools, and other natural settings this role coordinates services and planning across systems to promote stability, connection, and long-term success.

Therapists partner closely with youth, caregivers, and multidisciplinary teams to ensure everyone is aligned around shared goals, while building meaningful, culturally responsive relationships that honor each family’s values, strengths, and lived experiences.
