Carewell Supportive Care, a member of Empath Health is seeking a Palliative Care APRN. The APRN in the Community-Based Palliative Care Program provides comprehensive, patient‑centered care in collaboration with the Medical Director and interdisciplinary team. This role focuses on assessing complex needs, managing symptoms, and supporting patients and families through serious illness.
The APRN develops and evaluates individualized care plans, promotes self‑care, and ensures alignment with patient and family goals. Standard hours are Monday–Friday, 8:00 AM–5:00 PM, with participation in a rotating Saturday schedule that replaces a weekday.
