You’ll combine patient education, population health outreach, program development, and team support to help patients make meaningful lifestyle changes. This role is an opportunity to grow a health coaching program that reaches patients across the lifespan and strengthens the health of our community. What You’ll Do ▶ Develop engaging, evidence-based curriculum for pediatric and adult healthy living classes ▶ Facilitate interactive group education sessions both virtually and in person ▶ Analyze population health data to identify care gaps and support targeted outreach ▶ Provide front-line support and guidance to Health Coaches ▶ Serve as a subject matter expert for Health Coach policies, procedures, and program improvements ▶ Provide individual, telephone, virtual, and group patient education ▶ Case manage patients participating in preventative care and chronic disease programs, including diabetes ▶ Complete program reporting and collaborate with PCHG leadership and community partners What’s In It for You ▶ Mission-driven work focused on prevention and healthier communities ▶ Opportunity to develop and grow health education programs ▶ Collaboration with an interdisciplinary Care Team ▶ Opportunities to support chronic disease management and population health ▶ Professional growth through education, outreach, and program leadership ▶ Meaningful work where your patient education skills create lasting impact Qualifications ▶ Associate degree in public health, health education, nursing, diabetes peer education, or certification as a Community Health Worker required ▶ Bachelor’s degree preferred ▶ At least 1 year of patient education experience required; 3 years preferred ▶ Two years with Premier Community Health.

• Ability to lift 20 lbs. regularly and 30–50 lbs. occasionally
• Ability to sit for extended periods
• Travel between health centers to provide coverage and support
• Direct exposure to computer screens
