Direct Support Professional - Community Facilitator
Job description
The Direct Support Professional, also known as a Community Facilitator (CF) plans, coordinates, and provides individualized support to clients. Clients’ needs and preferences dictate the amount of support hours provided by the CF and when that support occurs. The CF works well with TMI employees, clients’ circle of support (family members, friends, community members, etc.)
, community resources, the Department of Rehabilitation, and the Regional Center. Also, the CF works independently and as a team member.
