About the Role
We are seeking an experienced Case Manager to oversee and coordinate supports for children and young people under both Child Safety and NDIS frameworks. This role is critical in ensuring compliance, strong stakeholder engagement, and positive client outcomes across our Sunshine Coast operations.
Key Responsibilities
Manage a caseload of Child Safety and NDIS participants Develop and review care plans, support plans, and risk assessments Coordinate SIL and out-of-home care placements Liaise with Child Safety, families, support coordinators, and allied health Monitor NDIS funding utilisation and prepare review reports Ensure compliance with NDIS Practice Standards and Child Protection legislation Lead case meetings and multidisciplinary reviews Oversee incident reporting, risk management, and safeguarding About You Minimum 2–3 years’ experience in Child Safety, Out-of-Home Care, or NDIS Strong understanding of NDIS framework and Child Protection requirements (QLD) Experience working with complex behaviours and high-support clients Excellent communication, organisation, and problem-solving skills Ability to manage competing priorities and maintain compliance Requirements Essential Requirements Relevant qualification in Social Work, Psychology, Human Services, or similar Current Blue Card (Working with Children Check) Current NDIS Worker Screening Clearance Valid Driver’s Licence and reliable vehicle Benefits What We Offer Competitive salary based on experience Career progression within a growing organisation Supportive leadership and structured systems Opportunity to make a meaningful impact in children’s lives Point Care Pty Ltd is an equal opportunity employer.
