Community Memorial Hospital is currently looking for a Primary Care Advanced Practice Provide for our Family Health Center in New Hartford to provide comprehensive, patient-centered care under the supervision of the Medical Director. The APP evaluates, diagnoses, treats, and manages acute and chronic conditions, promotes preventive health, and collaborates with the healthcare team to ensure high-quality care for all patients.
