Clinical Case Manager Registered Nurse

Job Description Summary ‎ Candidates in the Columbus region strongly preferred The Case Manager plays a vital role in delivering client-centered services that support patients across the continuum of care. This position partners with patients and their families to coordinate healthcare and connect them with appropriate home- and community-based services.

We are seeking a skilled and patient-focused Case Manager (RN) to join our growing team.

Key Responsibilities

Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
