Case Manager

As a Case Manager, you will play a vital role in supporting youth as they work toward achieving their personal, educational, and treatment goals. You will be responsible for developing, coordinating, and facilitating individualized case plans and treatment programs that meet the requirements of the referring agency while addressing the unique needs of each youth on your caseload.

Working closely with a team of dedicated professionals, you will help create meaningful opportunities for growth by providing individual counseling, teaching essential life skills, and guiding youth through their treatment journey.
