ACT Co-Occurring Disorders (COD) Specialist

Position Title: ACT Co-Occurring Disorders (COD) Specialist Department: Assertive Community Treatment (ACT) Reports To: ACT Team Lead and Director of Behavioral Health Services Position Summary The ACT Co-Occurring Disorders (COD) Specialist provides integrated treatment services to individuals with Serious Mental Illness (SMI) and co-occurring substance use disorders.

Services are delivered in community settings in accordance with ACT, DBHDS, DMAS, and TMACT standards.

Essential Duties and Responsibilities

• Conduct substance use screenings and assessments.
• Develop individualized treatment and relapse prevention plans.
• Utilize Motivational Interviewing, Harm Reduction, and Recovery-Oriented approaches.
• Provide integrated mental health and substance use interventions.
• Deliver services in homes, shelters, hospitals, and community settings.
• Coordinate care with medical providers, MAT programs, and community agencies.
• Participate in ACT team meetings and treatment planning.
• Provide education on addiction, recovery, and wellness.
• Participate in crisis intervention and outreach activities.
• Complete documentation in compliance with Medicaid and agency requirements.

Qualifications

• Bachelor's Degree in Human Services, Counseling, Psychology, Social Work, Substance Abuse Counseling, or related field required.
• Master's Degree preferred.
• CSAC, CSAC-A, LPC, Resident in Counseling, LCSW, or related credential preferred.
• Experience working with individuals with co-occurring disorders preferred. Work Schedule
• Full-Time
• Flexible schedule based on participant needs.
• Participation in ACT on-call and crisis coverage rotation as assigned.
