Intake Coordinator

Full-Time | In Office | Monday-Friday, 8:00 AM-5:00 PM As Faith Home Health and Hospice continues to grow, we are looking for a detail-oriented and compassionate Intake Coordinator to join our team. As an addition to our current team, this vital role serves as the first step in the patient care journey, helping ensure referrals, admissions, and communication processes are handled efficiently and accurately.

What You'll Do

• Verify insurance eligibility and coordinate patient admissions
• Answer incoming calls and manage fax communications
• Enter and process patient referrals within our EMR system
• Collaborate with clinical and administrative teams to coordinate services
• Create, update, and maintain department forms and documents
• File, organize, and maintain patient records
• Process and distribute Medicare-related paperwork
• Track physician orders and maintain compliance documentation
• Assemble patient admission packets
• Support quality assurance initiatives and regulatory compliance efforts What We're Looking For:
• Valid driver's license and reliable transportation
• Minimum of one year of home health experience preferred
• Proficiency with Microsoft Office applications, including Word and Excel
• Strong organizational skills and attention to detail
• Ability to manage multiple priorities while maintaining accuracy
• Excellent communication and customer service skills
• Availability to work alternating weekends (with a weekday off prior to the on-call weekend) Why Faith Home Health & Hospice?
