Medical Coordinator (Buffalo)

The medical coordinator provides clerical support to the nursing staff and direct support to the people served at the residential site. Promotes Person-Centered planning for people receiving services and their family members/guardians. Collaborates with the entire residential team including DSPs, clinicians, community agencies, and others involved in medical/nursing care as appropriate.

Provide oversight and guidance to direct care staff of designated shifts. Ensure completion of required duties for each shift.
