• Provides nursing services in the patient’s home in accordance with agency standards and applicable laws and regulations. Ensures that patients receive the highest possible quality of care.

Key Responsibilities

• Ensure quality and safe delivery of home health care services.
• Implement current nursing practice to meet the needs of patients.
• Fulfill additional performance responsibilities as assigned and as necessary to support patient care.
• Promote effective written and verbal communication with patients, families, and healthcare teams.
• Use equipment and supplies effectively and efficiently to provide optimal care.Qualifications: Required Education:
• Completion of a state-approved practical nursing program Preferred Education:
• Licensure in Practical Nursing Required Experience:
• Experience in providing nursing services in a home care setting
• Familiarity with applicable laws and regulations governing home health care
