Location: Hickman Hospital Department: Hematology And Oncology Weekly Hours: 40 Status: Full time Shift: Days (United States of America) Job Summary: As the CIN Care Navigation RN, you will coordinate and optimize care transitions for ACO and patients transitioning from acute care to post-acute care and ultimately, home .

You will track patients transitioning into post-acute care facilities to ensure continued quality care. You will work with post-acute providers to monitor and improve patient outcomes. The above summary is intended to describe the general nature and level of work performed by the positions. It should not be considered exhaustive.

REQUIREMENTS

Associate or bachelor’s degree in nursing. Skills: Must have knowledge of case management and community resources. Must be able to function independently and effectively in a fast-paced environment. Written and verbal skills are essential. Must be able to establish priorities and communicate effectively. Years of Experience: At least 2 years of previous case management, care navigation, or post-acute care experience.

License: Must have active and unencumbered RN in Ohio and Michigan - or be willing to obtain upon hire. PREFERRED REQUIREMENTS Education: Master’s degree in nursing or other relevant healthcare field. Skills: Microsoft Office competency Years of Experience: 2 years Pro.
