completing evaluations and re-evaluations; creating and updating individualized plans of care; providing therapeutic interventions and caregiver education; coordinating with the interdisciplinary team to maintain continuity; and completing accurate, timely documentation of visits, outcomes, and communication.

Required qualifications/skills:

• Current Arizona Occupational Therapist license in good standing
• Home health or comparable community-based experience preferred
• Strong clinical judgment, organization, and documentation accuracy
• Clear communication and ability to work independently
