We are looking for a compassionate, skilled Licensed Practical Nurse (LPN) to join our growing PACE (Program of All-Inclusive Care for the Elderly) team.

The Licensed Practical Nurse (LPN) provides direct participant care, supports providers during examinations and procedures, and assists with care coordination in a clinic setting. This role ensures efficient patient flow, delivers high-quality nursing care within scope of practice, and supports clinical operations to promote positive participant outcomes.

What You Bring Current and valid Licensed Practical Nurse (LPN) license in the State of Colorado, maintained throughout employment. Graduate of an accredited practical nursing program. Current and valid Colorado driver's license, adequate automobile insurance and unlimited access to reliable transportation. 1-2 years experience as an LPN, experience working in a clinic, outpatient or hospital setting preferred.

Strong clinical judgment and the ability to recognize and communicate changes in participant condition. Excellent communication and interpersonal skills.
