Case Manager

We are seeking a Case Manager within on our Campus to contribute to our mission and drive quality outcomes and patient satisfaction within out Skilled Nursing Home. The primary focus of this position is ongoing case management, including care coordination, care plan follow-through, resource navigation, coverage coordination, and discharge planning. The Case Manager provides backup assistance to the admissions team as needed; ownership of the admissions process and census development are not the primary responsibilities of this position.
