Job Responsibilities

Complete start-of-care admissions and supervisory visits Travel within the Treasure Coast as required Provide safe, effective nursing and personal care based on the Plan of Care Administer medications and perform skilled nursing tasks within scope of practice Assist with transferring, positioning, catheter care, and mobility needs Communicate promptly regarding changes in medical status or emergencies Coordinate care with the Nurse Supervisor / Clinical Manager Escort clients to and from appointments or activities as requested Conduct occasional client assessments and supervisory visits Demonstrate empathy, professionalism, attention to detail, and sound clinical judgment Requirements Active Registered Nurse (RN) license Level 2 AHCA Clearinghouse clearance TB test (within last 12 months) or Chest X-ray (within last 5 years) Home care experience Strong communication, teamwork, and customer service skills Ability to use an electronic health record (EHR) for documentation and care coordination Be part of a team that values clinical excellence, compassionate care, and caregiver support .
