  • Job Overview Schedule: Monday-Thursday (7AM-4:15PM) | Friday (7AM-3PM) As a Nurse Practitioner or Physician Assistant in our School-Based Health Program, you will provide compassionate, high-quality care to children in a school-based community clinic setting, along with family members of enrolled students. Working under the supervision of physicians, you will help make healthcare more accessible to students and families while supporting healthier futures. Key Responsibilities

Perform routine pediatric care in school-based community clinics, including medical procedures and drug treatments according to procedures, policy, and protocols. Assess, treat, and provide medical services to clinic clients. Initiate an appropriate assessment of the patient’s health status. Obtain patients’ health histories through interviews with patients and others and review of pertinent health records.

Perform physical examinations. Order pertinent diagnostic tests and procedures. Perform diagnostic and therapeutic procedures within the scope of practice, including venipuncture; intradermal tests; intramuscular and subcutaneous injections; application of dressings and bandages; administration of medications; cardiopulmonary resuscitation; auditory screening; visual screening; and aseptic techniques.

Collect specimens and carry out commonly performed blood, urine, and stool analyses and cultures. Interpret health information and establish diagnoses consistent with the accumulated data. Formulate treatment plans and ensure the timely provision of necessary healthcare services. Order necessary medications and other therapeutic equipment and supplies within the scope of practice.

Educate and inform patients regarding the nature of illness and the progress of evaluation and treatment. Educate patients regarding health promotion and disease prevention. Analyze accumulated health information, collect additional observational, interview, historical, physical examination, and diagnostic data, and modify treatment as appropriate.

Make health record entries that are legible, concise, and timely. Ensure evaluation of laboratory and other testing results, with appropriate follow-up of abnormal values. Consult with and refer complex and high-priority cases to the physician on duty, including complicated diagnostic problems, serious illness, and re-evaluation of chronic conditions.

Consult with clients by telephone, triage concerns, and refer clients to services as appropriate. Consult with colleagues, supervising physicians, specialists, and other health professionals as appropriate. Assist the Medical Director and Director of School-Based Clinics with the ongoing assessment of client services and report identified opportunities for improvement.

Work with the team on ongoing clinic development and implementation. Assist in patient education, preventive healthcare, and health maintenance concepts. Interview and advise patients regarding health and illness prevention, including FQHC-required voluntary family planning services and related preventive health services in compliance with funding requirements and state and federal laws, rules, and regulations.

Provide primary clinical therapy and treatment to individual and groups of patients. Advocate for the rights of patients. Participate in IDT. Participate in the Performance Improvement Program. Participate in formulating, interpreting, implementing, and evaluating objectives, policies, and procedures. Participate in committees that improve the quality of healthcare.

Participate in the collection and preparation of reports on patient care activities. Conduct audits related to advanced practice healthcare techniques and participate in quality assurance programs. Maintain current knowledge in the field of practice and inform staff of changes. Document all client care appropriately and in a timely manner.

Assist with school-based clinic registration activities and educational activities. May assist other providers in the future as needed due to clinic expansion. Participate in on-call duties as directed, if needed. Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to help ensure a professional, responsible, and courteous environment.

Promote effective working relationships and work effectively as part of a team to help the department meet its goals and objectives. Attend all required safety training and education programs and describe responsibilities related to general safety, department/service safety, and specific job-related hazards. Operate assigned equipment and perform all procedures safely as instructed.

Maintain the work area and equipment in the condition required by Legacy standards. Demonstrate proper body mechanics in all functions. Check the patient’s environment and equipment to ensure safety. Report safety concerns promptly to appropriate personnel. Follow Legacy’s exposure control plans for blood-borne and airborne pathogens.

Demonstrate knowledge of techniques, procedures, and the correct use of protective barrier equipment and Universal Precautions. Assure a safe environment by instituting appropriate control measures. Attend annual education programs. Perform other related work and duties as assigned.

Minimum Qualifications

Current Texas state licensure or eligibility for licensure and graduation from an accredited program as Physician Assistant NP: Current license from the Texas Board of Nursing and completion of program at an accredited school. Provider shall obtain, maintain and participate in the Medicare and Medicaid Programs, workers’ compensation, other federal and state reimbursement programs, and the payment plan of any commercial insurer, health maintenance organization, preferred provider organization, accountable health plan, and other health benefit program requested by Legacy.

Required Years of Experience PA: 1 year of medical experience in a physician’s office or equivalent combination of training and experience Desirable Education and/or Experience PA and NP: 2-5 years experience is preferred. PA and NP: School-based (SBH) clinic experience is highly desired. Required Knowledge, Skills, and/or Abilities Must be able to use electronic medical record with proficiency.

Ideal candidate is independent, enthusiastic and highly motivated. Must be able to manage multiple deadlines and prioritize. Desirable Knowledge, Skills, and/or Abilities Bilingual in Spanish is a plus.
