Caseworker Specialist II

Day-to-day responsibilities of this position: Coordinate Assessments and Support for individuals. Support Individuals to dictate their service plans. Complete field work assignments (throughout the state of Utah) to complete assessments used to support Persons initial Person Centered Support Plan (PCSP) and budgets. Provide consultation, make recommendations, give appropriate advice, and/or facilitate decisions.

Assess individual needs, develop and implement plans to meet those needs. Coordinate and refer services of community resources, placement providers, collateral agent.
