Job Summary

Provides social work services to patients and their caregivers in the home setting#to achieve the goals identified in the plan of care.# Communicates plan to team members. Identify and assist with community resource linkages.

Minimum Qualifications

Must possess a valid RI MSW license, valid driver#s license, current auto liability insurance and maintain current CPR certification. Master#s degree in social work. Success Factors: Minimum 3 years social work experience required within a home health care setting preferred. Experience working with medical social services and community resources.

Proficient in basic computer skills.
