Clinical Care Manager
Job description
The BEW (Be Into Health for Women) Project is a partnership between Housing Works and NYC DOHMH, supports Black and Latina women living with HIV in reengaging in HIV care and essential support services. The Clinical Care Manager works with an interdisciplinary team to assess client needs, coordinate care, develop health goals, and promote retention in care and self-management.
The ideal candidate has experience serving high-risk populations and strong cultural competency working with diverse gender identities and communities.
