Job description
The Community Habilitation Specialist works with individuals in a one to one or small group setting to learn or maintain the skills needed to live safely and more independently within the community. The community habilitation specialist works with individuals to make and keep friends, participate in community events/groups, learn socially appropriate behaviors, community safety, choice making and self-advocacy skills.
The community habilitation specialist works with the individual(s) to create a community-integrated person-centered service.
