Compass Park is a vast 320-acre community in the heart of Johnson County, adjacent to Indianapolis. MDS Coordinator – General Responsibilities Organizes, schedules, and ensures the timely initiation and completion of Minimum Data Set (MDS) assessments, other required assessments, and individualized resident care plans. Reviews MDS assessments and quarterly assessments for accuracy and completion, prepares Care Area Assessment (CAA) summaries, obtains required RN signatures for MDS assessments, and completes and signs all applicable forms within the scope of licensure. Monitors care plan content for accuracy, completeness, and compliance with resident needs, assessment findings, and applicable regulations. Ensures approved assessment criteria, tools, and procedures are followed in accordance with Nursing Department policies and procedures. Maintains current knowledge of changes in state and federal regulations affecting resident care, MDS assessments, reimbursement, and facility operations. Participates in facility surveys, audits, and regulatory reviews and assists with addressing identified deficiencies as needed. Monitors activities of daily living (ADL) coding and provide education and guidance to staff as necessary to maintain accurate and consistent documentation. Audits resident records and clinical documentation to identify opportunities for improvement, ensure accuracy, and support regulatory compliance. Collaborates with therapy services and the interdisciplinary team to ensure appropriate assessments, accurate documentation, and optimization of applicable reimbursement opportunities. Monitors and assists with the development and implementation of plans to improve facility Quality Measures and resident outcomes. Collaborate with nursing staff, physicians, therapy, social services, dietary activities, and other interdisciplinary team members to support comprehensive resident assessments and individualized care planning. Ensures MDS assessments and supporting documentation are completed accurately and within required regulatory time frames. Maintains resident confidentiality and promotes resident rights, safety, and quality of care in accordance with facility policies and applicable regulations. Performs other related duties and responsibilities as deemed necessary and completes assigned tasks within established guidelines and time frames. MDS Coordinator – Required Education, Experience, and Skills Must possess and maintain a current, valid Indiana LPN or RN license. Minimum of three (3) years of experience as an MDS Coordinator. Demonstrates knowledge of nursing assessments, techniques, and restorative procedures within the scope of practice. Knowledge of applicable state and federal regulations governing long-term care, MDS assessments, and resident care planning. Strong organizational, communication, documentation, and time management skills. Ability to review clinical documentation for accuracy, identify opportunities for improvement, and meet required assessment deadlines. Ability to collaborate effectively with nursing staff and interdisciplinary team members. Ability to maintain confidentiality and comply with facility policies and procedures.
