POSITION SUMMARY

Performs and assists with client admissions and client’s yearly re-evaluations. Provides training to caregivers on peer-to-peer level. Also provides personal healthcare and related services to the client in their place of residence. Assists in providing a safe and clean environment, work cooperatively with patients and family, and share observations and problems with the supervisor.

This position functions under the direction, instruction and supervision of the PAS Director. Works closely with Director of PAS and reports any changes to office/Director.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned. Assists with HR compliance (evals, car insurance, etc.) Provides counseling to caregivers on attendance, tardiness, etc. Handles scheduling as needed. Participates in on-call rotation.

Documents all training, orientation, complaints, and visits. Provides caregiver peer-to-peer training. (Assist with Relias, clocking in/out, care plans, Paycor) Travels to multiple client homes daily. Facilitates orientation of newly hired caregivers. Assists with supervisory visits. Works closely with PAS Director to ensure caregivers are educated and understand company policies and procedures.

Ensures caregivers know how to read/access their client care plan. Assists Caregivers with clocking in, clocking out and entering task codes. Assists with client admissions and filling out documentation. Other duties as assigned.

EDUCATIONAL REQUIREMENTS AND EXPERIENCE Supervisory experience preferred. Valid Texas CNA license highly preferred. KNOWLEDGE, SKILLS, AND ABILITIES Language Ability: Ability to communicate effectively in writing and speaking the primary language of the residents. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.

Cognitive Demands: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Driving Requirements: Must have reliable transportation and the ability to drive extensively in the geographic area using personal vehicle.

Must have valid Texas driver’s license and automobile liability insurance acceptable to the company and our insurers.
