Join our Assertive Community Treatment (ACT) team and help individuals with serious mental illness live independently, stay out of the hospital, and build meaningful lives in their communities. This is not traditional outpatient therapy. ACT is hands-on, team-based, and deeply impactful work—where every day is different, and your support meets clients where they are.

What You’ll Do

• Provide community-based mental health services in homes, shelters, and other real-world settings
• Work as part of a multidisciplinary team (psychiatry, nursing, case management, peer support)
• Share responsibility for a team caseload—no isolated practice Support clients with: o Symptom management o Crisis intervention o Housing stability o Substance use recovery o Daily living skills Why This Role Is Different

• Lic ensed Professional (LMSW, LGPC, LCPC, LCSW-C)
• Strong communication and teamwork skills
• Comfort working in community settings and adapting to changing situations
