POSITION SUMMARY

Our Wellness Nurse (RN) plays a vital role in ensuring the health and well-being of our residents. In collaboration with the Health and Wellness Director, this role provides high-quality care through health assessments, medication management, and coordination with residents, families, and the care team.

Responsibilities include

monitoring resident health, addressing changes in condition, and promoting a safe and supportive environment that aligns with our mission to provide exceptional care.

KEY RESPONSIBILITIES

Delegations. New move in assessments. Initial service plans. Significant changes in condition/Return to community assessments and service plan updates/ISPs. Dashboard review – past due assessments, assessments pending approval, missed meds, meds unavailable, etc. Third medication check. Review and sign off all service plans.

Report to community for emergency situations. Maintains a high degree of resident satisfaction through consistently delivering high-quality services. Understand the community’s care regulations along with state-specific regulations. Maintains current departmental policies, procedures, and licenses per company, Federal, State, and local requirements.

Responsible for creating and maintaining an atmosphere of stability where the personal dignity of residents is supported. Develops and maintains a positive image with the community. Interact with residents' families, physicians, and medical personnel as needed. Adheres to all policies and procedures of Cogir Senior Living.

Other responsibilities and duties as assigned.

Requirements

CANDIDATE QUALIFICATIONS Education and Certifications: A valid, incumbent nursing license (RN) is required. High School Diploma or equivalent. An associate degree or higher in nursing is preferred, or a combination of education and experience. Additional coursework in management is an asset. Current First Aid and CPR license or ability to obtain.

Experience, Competencies, and Skills: At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies. At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred. Experience with memory care is a plus.

Positive attitude, empathy, patience, and commitment to treating our residents with dignity and respect. Ability and willingness to participate in on-call rotation.
