Physical Therapist Assistant (PTA) – Home Health (Per Diem) Location: Serving Mahoning, Trumbull, and Columbiana Counties Physical Therapist Assistant (PTA) – Home Health (Per Diem). Per Diem Southwoods Health at Home, a division of Southwoods Health, is seeking a patient-centered Physical Therapist Assistant to join our home health team.

In this role, you will provide vital therapy treatments under the direction of a Physical Therapist, helping patients restore function and regain independence in the comfort of their own homes.

Essential Duties

Clinical Care: Implement medically prescribed physical therapy treatments according to the individualized plan of care developed by the Physical Therapist. Patient Progress: Monitor and record patient responses to treatment, including strength, motor development, and functional capacity. Collaboration: Work closely with the Physical Therapist and the multidisciplinary team to suggest treatment revisions and integrate care.

Education

Empower patients and their families by providing instruction on home exercise programs and post-operative recovery protocols. Documentation: Maintain accurate, timely patient records in accordance with Medicare and facility standards.

Qualifications

Education

Graduate of an accredited Physical Therapy Assistant program. Licensure: Active Physical Therapist Assistant (PTA) license in the State of Ohio. Certification: Current BLS certification through the American Heart Association (AHA). Skills: Strong interpersonal skills and the ability to provide compassionate care independently in a home setting.
