An Integrated High Needs Coordinator provides timely, evidenced-based clinical and non-clinical support services those with severe and/or chronic disabilities that may limit a person's ability to do the tasks related to daily living. Services will include intakes, assessments, treatment planning, psychiatric support, and group interventions.
