Integrated Behavioral Health Provider

Utilizes a variety of person centered, culturally sensitive of treatment approaches to work with a diverse primary care population. Service goals include: providing brief (5-7 visits) solution oriented behavioral health therapy to clients to improve their functioning for physical and behavioral health conditions, crisis management, behavioral health case management, care coordination and consultation.

Offer referrals and support to transition patients to a longer term provider if needed after 5-7 visits in order to continue to be able to accept warm hand offs and referrals from primary care providers at GVH Family Medicine Clinic locations.
