Case Manager Los Angeles, CA | Full-Time | Field-Based | $25.00 - $29.15 per hour Kedren Community Health Center, Inc. is seeking a compassionate and experienced Case Manager to provide coordinated services and support to individuals with serious and persistent mental health needs. This position works closely with clients, families, multidisciplinary treatment teams, and community agencies to assess client needs, coordinate services, connect clients with appropriate resources, and support positive outcomes.

The ideal candidate will have experience working in behavioral health or human services and a strong understanding of community-based resources and client advocacy.

Key Responsibilities

Assess clients' psychosocial needs and community functioning to support individualized care planning. Coordinate services and monitor client progress toward established goals. Connect clients and families with appropriate mental health, social service, and community resources. Advocate on behalf of clients and families to help address barriers to care and access needed support services.

Collaborate with multidisciplinary treatment teams to promote coordinated and effective client care. Serve as a liaison between Kedren and community agencies, programs, and service providers. Conduct client interviews and use appropriate case management techniques to identify needs and available resources. Maintain timely and accurate documentation of client services, activities, and progress.

Monitor coordinated service plans and facilitate the exchange of information necessary to support client goals. Maintain client confidentiality and adhere to applicable legal and ethical requirements. Identify concerns that may affect the quality of services and collaborate with appropriate staff to develop solutions.

Communicate professionally and effectively with clients, families, treatment teams, and community partners. Assist with special projects and other duties as assigned.

Qualifications

Bachelor's or Master's degree in Social Work, Psychology, Human Services, or a related field. Minimum of 2 years of human services experience preferred , including experience as a Case Manager, Social Services Case Worker, Educational Psychologist, or in a related role. Experience working in a psychiatric or behavioral health environment with individuals who have serious and persistent mental health needs preferred .

Knowledge of community resources and coordinated mental health services. Understanding of confidentiality requirements and legal and ethical standards related to client information. Knowledge of case management principles, psychosocial assessment, and interviewing techniques. Strong verbal and written communication skills.

Strong interpersonal, teamwork, and problem-solving skills. Ability to work effectively as part of a multidisciplinary treatment team. Ability to maintain accurate and timely client documentation and records. Comfortable providing services in field and community-based settings .
