Position Summary

We are seeking a Marketer to join our team and play a dual role in referral source management and patient intake . This position relies on relationship-building skills to ensure we maintain close relationships with referral sources and build strong relationships with new referral sources. Key Responsibilities

Manage incoming referrals from hospitals, skilled nursing facilities (SNFs), case managers, and other partners. Generate new referral sources. Perform patient intakes and execute service agreement contracts as needed. Coordinate care initiation promptly to reduce delays and improve patient experience. Maintain accurate referral and intake trackers.

Build and maintain strong relationships with referral sources through regular communication and check-ins. Generate new referrals from established referral sources. Identify opportunities for growth and assist with marketing initiatives as needed. Serve as a liaison between referral sources, patients, and internal care teams.

Qualifications

Strong organizational and time-management skills. Excellent communication and interpersonal skills. Ability to work independently and manage multiple priorities. Previous experience in home health, personal care, or case management preferred. Marketing or referral development experience is a plus (but not required). Why Join Stonebridge?

Compensation
