Case Manager - Managed Care

The Managed Care Case Manager conducts intake, assessment, care planning, coordination, and follow-up for individuals needing long-term services and supports. This role determines program eligibility, develops and monitors person-centered support plans, coordinates services, and advocates for vulnerable populations.

Responsibilities include

maintaining records, participating in community health initiatives, and collaborating with internal teams and external providers. Work involves office, community, and in-home settings with regular travel. A background in social work, nursing, or a related field and MnCHOICES certification (or willingness to obtain) is required.
