About Us
Noble Community Clinics is a Federally Qualified Health Center (FQHC) serving Wisconsin communities for over 60 years. Our mission is to ensure accessible, high-quality care that strengthens individuals, families, and communities. What You’ll Do This role provides psychotherapy and substance use disorder (SUD) counseling as part of our integrated Behavioral Health team.
Key Responsibilities
Provide individual, couples, family, and group therapy using evidence-based approaches Complete mental health and AODA assessments, diagnostic evaluations, and treatment plans Determine SUD treatment needs using ASAM criteria Facilitate individual and group SUD counseling sessions Coordinate care with primary care, MAT prescribers, county agencies, and community partners Maintain timely and accurate documentation in the EMR (EPIC) Participate in team meetings, case staffing, and quality improvement initiatives Educate patients and families on diagnosis, treatment, and recovery options Conduct outreach activities and rotate in an on-call schedule Work may occur in clinic, office, telehealth, and community settings, with some local travel required.
