Job description
Job Description: Campus Admissions & Clinical Documentation Coordinator (RN/LPN) | Admissions, Care Transitions & Clinical Documentation Covenant Skilled Nursing & Rehab at Wellspring
- Saginaw | Full-Time, on-site | Hourly $33.00 - $38.00 You became a nurse because you wanted to help people during some of life's most vulnerable moments. You understand that a person's experience with healthcare begins long before the first treatment is provided. It often starts with a phone call, a conversation with family, a difficult transition, or the uncertainty of what comes next. You know how much it matters when someone is welcomed with compassion, guided with confidence, and reassured that they are in capable hands. At Wellspring Skilled Nursing & Rehab, we're looking for a Campus Admissions & Clinical Documentation Coordinator who understands that every admission is more than paperwork—it's the beginning of someone's journey with us. As the first point of connection for many residents and families, you'll play a vital role in creating a smooth, supportive transition into our community. You'll combine your clinical knowledge, communication skills, and attention to detail to ensure residents receive the care they need while helping families navigate an often emotional process. At the same time, you'll help ensure the accuracy of clinical documentation, insurance authorizations, and communication processes that support quality care throughout a resident's stay. Most importantly, you'll help residents and families feel welcomed, informed, and cared for from day one. What Your Impact Will Look Like You'll help families move through the admissions process with confidence, answering questions and ensuring they feel supported during an important life transition. You'll work closely with centralized admissions, nursing leadership, therapy teams, and other departments to coordinate admissions, discharges, and bed availability so residents can move into the community smoothly. You'll prepare for each admission by ensuring rooms, equipment, and campus teams are ready to welcome new residents. You'll conduct tours and serve as an ambassador for the community, helping prospective residents and families envision the care, support, and services available to them. You'll use your clinical knowledge to support documentation processes, insurance authorizations, Medicare certifications, and communication with MDS teams to ensure accurate and timely reporting. You'll collaborate with interdisciplinary teams to ensure important clinical changes are communicated and documented appropriately throughout a resident's stay. Some days you'll be helping a family complete admission paperwork and answering their questions. Other days you'll be coordinating insurance approvals, reviewing documentation, participating in care conferences, or partnering with team members to ensure every transition is handled with excellence. Every day, you'll help create a positive experience for residents and families while supporting the clinical and operational success of the community. The Professional We're Looking For You believe that great healthcare starts with great communication. You understand the importance of balancing compassion with organization and attention to detail. You enjoy building relationships and helping people feel comfortable during times of uncertainty. You are highly organized and able to manage multiple priorities while maintaining accuracy. You appreciate the connection between quality documentation and quality care. You enjoy collaborating with interdisciplinary teams and helping important processes run smoothly. You take pride in creating positive first impressions and delivering exceptional customer service.
