We provide evidence-based, holistic care through a full continuum of services, from residential treatment to outpatient services and recovery support. We are seeking a compassionate and dependable Advanced Practice Registered Nurse (APRN) to join our Team in a full-time, remote telehealth position. The APRN plays a vital role in providing comprehensive, patient-centered healthcare, including Medication-Assisted Treatment (MAT), through virtual healthcare services.
This position is responsible for assessing and diagnosing patients, developing individualized treatment plans, ordering and interpreting diagnostic testing, prescribing medications, and managing acute and chronic health conditions. Working collaboratively with our medical and clinical teams, the APRN supports each patient's physical and behavioral health needs while promoting treatment engagement, continuity of care, and long-term recovery success.
JOB SPECIFIC FUNCTIONS: Provide comprehensive healthcare services through telehealth, including patient assessments, diagnosis, treatment, and ongoing medical management. Assess new admissions to identify medical conditions, withdrawal symptoms, and healthcare needs that may impact recovery. Provide Medication-Assisted Treatment (MAT), withdrawal management, and medication monitoring in accordance with established protocols.
Diagnose and manage acute and chronic medical conditions, including prescribing medications, ordering diagnostic testing, and coordinating referrals. Develop individualized treatment plans, monitor patient progress, and adjust treatment as needed. Collaborate with medical and clinical teams to address patients' physical, mental, and behavioral health needs.
Educate clients and staff on medications, disease prevention, chronic conditions, and overall wellness. Review medical records, conduct peer reviews, and participate in peer-to-peer evaluations as needed. Evaluate patients requiring medical triage and determine appropriate treatment or referrals. Maintain accurate, timely, and confidential medical documentation in accordance with HIPAA and organizational requirements.
Support medical policies, medication management procedures, infection control, and established standards of care. Coordinate with pharmacies, healthcare providers, and medical facilities to ensure continuity of care. Participate in required staff meetings and assist with additional medical responsibilities as directed by the Director of Medical Operations.
Ensure compliance with applicable healthcare regulations, telehealth requirements, and Isaiah House policies and procedures.
Requirements
Master's degree or higher in Nursing from an accredited program. Current, unrestricted Advanced Practice Registered Nurse (APRN) license in Kentucky. Current national certification in an applicable advanced practice nursing specialty. Active DEA registration required. Current prescriptive authority in accordance with Kentucky requirements.
Experience in primary care, addiction medicine, behavioral health, or substance use disorder treatment preferred. Knowledge of Medication-Assisted Treatment (MAT), withdrawal management, and substance use disorders. Strong clinical assessment, diagnostic, and decision-making skills. Proficiency with electronic medical records (EMR) and telehealth technology.
Excellent communication, organizational, and documentation skills. Ability to work independently in a remote setting while collaborating with a multidisciplinary healthcare team. Understanding of HIPAA, patient confidentiality, and applicable healthcare regulations. Compassionate, professional, and nonjudgmental approach to patient care.
Ability to maintain a private, secure workspace and reliable internet connection for telehealth services. PHYSICAL REQUIREMENTS: Ability to sit for extended periods while conducting telehealth appointments and completing documentation. Frequent use of computers, keyboards, and other electronic equipment. Ability to communicate effectively with patients and staff through virtual platforms and telephone.
Sufficient visual and auditory ability, with or without reasonable accommodation, to perform essential job functions. Ability to maintain concentration and attention to detail while managing multiple patient needs. Ability to perform essential job functions in a remote work environment, with or without reasonable accommodation.
