We are recruiting for a full-time Support Coordinator

• ID. This position plays a vital role in improving the lives of individuals with intellectual disabilities by connecting them to the supports they need to live independently and thrive in the community.

Responsibilities

Provide case management and service coordination for children and adults with intellectual disabilities, assist individuals in accessing medical, psychiatric, social, educational, vocational, and residential services, and conducting home visits and comprehensive assessments.

Requirements

One ( 1) year experience working with individuals with ID/DD diagnosis. Valid Drivers License with a point balance of -6 or higher. Preferred Bachelor's in human services . Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.Our Agency maintains a drug-free workplace.
