Admissions Coordinator Nurse (Full-Time, Exempt) Hours: Monday

  • Friday, On-Call Rotation Required POSITION SUMMARY: Responsible for referral/resource relationship building, determining financial viability and proper placement of each admission based on medical history and factors, promoting a positive image of Woodside Manor and Woodside Villa to the community, and providing accurate and timely communication to potential admissions, referral sources, and appropriate facility team members. Provides administrative support for Woodside Manor and appropriate staff using organizational, relational, and computer skills to meet the needs and expectations within the assisted and independent living continuum. This position functions as a resource person for the operation of the administrative office and its related services.

ESSENTIAL FUNCTIONS

Greets people entering the establishment, communicates with residents, family members, or individuals to answer questions or direct their inquiry to the proper personnel. Makes determinations regarding admission to the facility based on financial appropriateness. Determines appropriateness of admissions to the proper facility/setting on the campus.

Responds to inquiries from families, hospital discharge planners, and other referral sources by providing information about the facility, including tours. Communicates effectively with prospective residents, families, coworkers and the public for accurate and timely information. Maximize occupancy levels at assisted living facilities across the campus.

Maintain apartment waiting lists, making offers to prospective residents when appropriate. Prepares admission paperwork and other data for assisted living & independent living facilities across the campus. Admits incoming residents to the assisted living facilities. Evaluate and maintain the Resident Individual Service Plans Manages Resident Concierge position including hiring, scheduling, providing support and ongoing training.

Updates applicable reports regarding projected admissions and apartment status. Processes and prepares correspondence; assembles packets and other projects Prepares occupancy reports. Provides guidance to families during vacating process. Performs marketing activities with continued focus on maintaining full occupancy Supports the Mission, Vision, and values of Woodside Senior Communities Is knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all residents/tenants in a safe, secure environment Observes care in ensuring the confidentiality of all data, including resident, employee, and operations data as required by HIPAA/HITECH regulations Abides by all applicable statutes and regulations through compliance with the Woodside Corporate Compliance and Ethics Program Completes requirements for in-service training Completes other tasks and duties as assigned BENEFITS: Medical, Dental, and Vision Insurance Accident / Critical Illness / Hospital Indemnity Insurance Company paid Life Insurance Company paid Long-Term Disability Voluntary Short-Term Disability Accrued Paid Time Off (PTO) 403(b) Retirement Plan with Employer Match Sick Time EDUCATION AND EXPERIENCE: Graduate of an accredited school of nursing Current/valid State of Wisconsin License and CPR certification required Must have Microsoft Office skills Excellent communication and organizational skills required.
