What You'll Do

Complete OT evaluations in patients' homes Develop individualized plans of care Help patients improve independence with activities of daily living Address dressing, bathing, grooming, toileting, meal preparation, and functional mobility Provide therapeutic interventions based on patient goals Assess home safety and fall risks Recommend adaptive equipment and home modifications Educate patients, families, and caregivers Monitor progress and adjust treatment plans Complete reassessments and discharge planning Coordinate with the interdisciplinary team Complete timely documentation using WellSky What We're Looking For Active Maryland Occupational Therapist License Required Graduate of an accredited Occupational Therapy program Reliable transportation Strong clinical and communication skills Ability to independently manage home health visits Home Health Experience Preferred, but Not Required WellSky Experience is a Plus Why Home Health OT?
