What You'll Do

Complete occupational therapy evaluations in patients' homes Develop individualized plans of care Help patients regain independence with activities of daily living Address bathing, dressing, grooming, toileting, meal preparation, and other daily activities Provide therapeutic interventions to improve functional ability Assess home safety and recommend adaptive equipment or modifications Educate patients, families, and caregivers Monitor progress and modify treatment plans as needed Complete reassessments and discharge planning Collaborate with the interdisciplinary care team Complete accurate and timely documentation using Well.

Sky What We're Looking For Active Maryland Occupational Therapist License Required Graduate of an accredited Occupational Therapy program Reliable transportation Strong clinical, communication, and organizational skills Ability to work independently in patients' homes Home Health Experience Preferred, but Not Required Well.
