Licensed Clinical Social Worker (LCSW) Community Health Center- Full Time

Summary of Position: The LCSW identifies, triages, and manages patients with medical and behavioral health conditions within Barton Community Health Center and Barton Psychiatry & Mental Health offices. They will provide skill training through psycho-education strategies and will provide brief interventions that are problem-focused and based on goals set in partnership with the patient.

The LCSW provides patient-centered behavioral health services, in collaboration with the Patient Centered Medical Home (PCMH), to patients at Barton Community Health Center (BCHC) with support from the medical director and manager.
