About the Role
We're seeking a compassionate and detail-oriented MDS Coordinator RN (LPN) to join our team in New London, Connecticut. As a key member of our interdisciplinary team, you'll be responsible for coordinating the care planning process for our residents, ensuring that their needs are met and that their goals are achieved.
You'll work closely with our nursing staff, therapists, and other healthcare professionals to develop and implement individualized care plans that promote optimal health and well-being.
Responsibilities
