Speech Language Pathologist (SLP) – Home Health | Per Diem North Bergen, NJ | Serving Hudson County

We are currently seeking an experienced Speech Language Pathologist (SLP)to join our growing team on aper diem basis supporting adult and geriatric clients throughout Hudson County, NJ.

What You’ll Do

As a Home Health Speech Language Pathologist, you will:

• Provide in-home speech therapy evaluations and treatments for adult and geriatric clients
• Assess, diagnose, and treat speech, language, communication, cognitive, and swallowing disorders
• Develop individualized treatment plans in collaboration with physicians and care teams
• Modify care plans as patient needs evolve
• Educate patients and caregivers to promote safety, independence, and improved quality of life
• Collaborate closely with nurses, therapists, and interdisciplinary team members
• Complete timely electronic documentation using a tablet-based EMR system

Qualifications

• Current NJ Speech Language Pathologist license required
• Certificate of Clinical Competence (CCC-SLP) preferred
• Home health experience preferred but not required
• Strong communication and organizational skills
• Comfortable with electronic documentation/EMR systems

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:

$65.00 - $75.00 per point
