net Job Description Summary The RN plans, organizes, and directs hospice care and coordinates services to meet the needs of patients and their families in their homes and communities. Essential Job Functions/Responsibilities:

• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
• Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
• Completes, maintains and submits accurate and relevant clinical notes in Homecare. Homebase (Pointcare).
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Teaches the patient and family/caregiver self-care techniques as appropriate.
• Attends interdisciplinary group meetings.
• Fulfills the obligation of requested and/or accepted case assignments. Work Location: On the road/ Field Service Coverage Area: Victorville and surrounding cities Position Qualifications
• CA RN License
• CPR/BLS certification
• CA Driver License
• Hospice: 1 year (Required)
• Compassionate & Caring
• Homecare. Homebase (Pointcare) Experience
