Position Summary

This role is responsible for assuring the receipt of high quality, cost-efficient care for all patients who seek services at Mt. Ascutney#Hospital; the oversight of social work, case management,#and discharge planning; developing expected patient outcomes, defining appropriate resource utilization and working collaboratively with providers to set goals; assuring the department#s work a#hospital compliance is in line with federal and state regulations, reimbursement guidelines as well as the policies and procedures of Mt.

Ascutney#Hospital.

Qualifications

Graduate of an accredited school of nursing or completion of a Master#s Degree Program in Social Work. Licensure # Certifications Current RN licensure in the State of Vermont (for RN applicants).
