LPN- Home Health

Schedule: Guaranteed Hours: Full-Time | 40 Hours per Week guaranteed (8 hours/day). Shift Schedule: Day Shift | 8:00 AM – 4:30 PM . Weekend Requirements: Every 3rd weekend required. On-Call Requirements: None listed for this need (must maintain general flexibility for schedule demands). Role Overview: Seeking an experienced Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) for a temporary Home Health contract assignment in Pittsfield, MA .

Operating independently in the field, the LPN provides comprehensive skilled nursing care, patient assessment, and health promotion directly in patient homes. The role requires clinical problem-solving, home care regulatory knowledge, reliable transportation in all weather conditions, and flexibility to manage home visit schedules.

Key Responsibilities

& Scope of Practice: Direct Home Skilled Care: Deliver in-home nursing interventions to manage acute and chronic illness, promote health restoration, prevent hospital readmissions, and facilitate patient rehabilitation. Clinical Evaluation & Interdisciplinary Care: Evaluate individual and family responses to nursing interventions; collaborate with multidisciplinary providers, community resources, and home health agency leadership.

Independent Caseload & Time Management: Exercise independent clinical judgment to organize, self-schedule, and execute home visit caseloads within established agency protocols and regulatory standards. Documentation & Regulatory Compliance: Maintain detailed clinical records, execute data collection, and adhere to home health reimbursement, OASIS, and regulatory guidelines.

Safe Travel & Emergency Readiness: Drive safely to home visits across the service region in varied/inclement weather; maintain emotional stability and critical thinking during medical emergencies.

Qualifications

& Submission Requirements: Mandatory Clinical Credentials & Experience: Licensure: Active Massachusetts LPN License in-hand at time of profile upload [REQUIRED] (facility will not pend licensing) . Basic Life Support: Active AHA BLS or American Red Cross BLS certification [REQUIRED] . Home Health Experience: Demonstrated Home Health nursing experience [REQUIRED] .

Acute Care Background: Minimum 1 year of acute nursing care experience [REQUIRED] (hospital-based preferred) . Vehicle & Driver Credentials: Valid Driver’s License , active auto insurance, and reliable personal vehicle for daily home visit travel [REQUIRED] . Specialized Experience: Certified Home Health background with OASIS proficiency [HIGHLY PREFERRED] ; infusion therapy experience [PREFERRED] .
