Position: Licensed Practical Nurse (LPN) or LPN Weekend Warrior Location: Edgerton Care Center – Edgerton, WI (on-site position) Status: Full-time | Part-time | Casual Schedule: Days | PMs | NOCs | Weekends – True Flexible scheduling - you choose what fits your schedule _________________________ Job Summary: The LPN must be a clinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care.

Effectively interacts with patients/residents, families, responsible party, and all health team members. _________________________ Compensation: LPN - $28.00 – $32.48 base hourly wage, dependent on experience PLUS $2.00 per hour PM and $2.50 per hour NOC shift differentials

• LPN Weekend Warrior - $28.00 – $44.98 base hourly wage, dependent on experience Weekend Warrior Program – choose one of the following 3 levels: Select Level: base hourly wage +$5.00/hour (every other weekend) Pro Level: base hourly wage +$7.00/hour (3 weekends per month) Elite Level: base hourly wage +$10.00/hour (4 consecutive weekends; 5th weekend off) PLUS shift differentials - $2.00 per hour PM shift and $2.50 per hour NOC shift Benefits: Health insurance options designed for flexibility Dental and Vision insurance Company-paid life insurance 401(k) with employer match PTO, True Flexible Scheduling, and Shift Differentials Tuition reimbursement and continuing education Employee Assistance Program (EAP) Referral bonuses, employee discounts, early wage access Benefits vary by number of hours regularly scheduled and employment status _________________________ Key Responsibilities: Provide safe, effective nursing care per care plans and physician orders Administer medications and treatments accurately Assess residents and report changes promptly Document care in the EMR Monitors performance and mentor CNAs Ensure infection control and safety compliance Completes all other duties as assigned. _________________________ Qualifications: Required: Active LPN license in state of Wisconsin or licensed in any U.S state and be eligible for Wisconsin licensure by reciprocity.
