Registered Nurse (RN) / Licensed Practical Nurse (LPN) – Field-Based Home Care

$90,000–$105,000 annually | Monday–Friday | No Weekends | Mileage Reimbursement

Abernathy Home Care is seeking a compassionate and dependable RN or LPN to join our team in a full-time, field-based position. In this role, you’ll travel to clients’ homes to complete assessments and supervisory visits while helping seniors and adults continue living safely and independently in their communities.

Home care and ASAP experience are preferred, but not required. New graduates are encouraged to apply.

We work closely with Aging Services Access Points (ASAPs), case managers, families, caregivers, and other community partners to coordinate services and provide dependable, person-centered care.

About the Role

This is a full-time, field-based nursing position with daily travel to client homes. The RN/LPN will conduct client assessments and reassessments, complete caregiver supervisory visits, monitor changes in client needs, and ensure services are being delivered safely and appropriately.

You'll work independently in the field while remaining closely connected with our clinical and operations teams, case managers, caregivers, clients, and families.

This is an active position that involves regular travel and frequent walking throughout the workday.

Compensation & Benefits

• $90,000–$105,000 annually, based on experience
• Mileage reimbursement
• Full-time, Monday–Friday schedule
• Flexible schedule options: 7:00 AM–3:00 PM or 8:00 AM–4:00 PM
• No weekends
• Stable daytime schedule
• Supportive clinical and operations team

Key Responsibilities

• Conduct in-home client assessments and reassessments
• Complete supervisory visits for HHA/PCA caregivers
• Monitor client needs, conditions, and changes in care
• Ensure services are provided safely and in accordance with applicable program requirements
• Communicate and collaborate with ASAP case managers, caregivers, clients, and families
• Maintain accurate and timely clinical documentation
• Communicate changes, concerns, and recommendations to the appropriate members of the care team
• Help support quality, continuity, and compliance across client services

Requirements

• Active RN or LPN license in Massachusetts
• Home care experience preferred, but not required
• New graduates are encouraged to apply
• ASAP/community-based care experience strongly preferred
• Strong assessment, communication, and documentation skills
• Comfortable working independently in the field
• Reliable transportation and willingness to travel to client homes daily
• Comfortable with frequent walking throughout the workday
• Ability to safely lift, carry, and move up to 50 lbs
• Bilingual Spanish or Haitian Creole preferred, but not required
