Responsibilities

Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming Assist with mobility and transfer patients, as needed Administer medication and maintain accurate records Monitor and report any changes in patient condition to the Nurse, Physician, and family members Participate in care planning and development of individualized care plans Maintain a clean, safe, and comfortable environment for patients Collaborate with the healthcare team to ensure continuity of care About the Role: As a Home Health Aide with Constant Care Health Services LLC, you will have the opportunity to make a meaningful difference in the lives of our patients.

Requirements
